Use the worksheet for your own planning and reflection. For a formal submission, use the current ICE or IStructE form. Completing a worksheet does not certify competence.
What to record
Project
A code name if the real one is confidential.
Role and dates
Your role, not the project's title, and the period.
Employer and setting
Consultant, contractor, client, asset owner.
Stage
Feasibility, design, construction, commissioning, operation.
My responsibility
What was yours to get right, and the limit of your authority.
Decision
What you decided. This is the field you will use when writing.
Options rejected
What else could have been done, and why it was not.
Result and reflection
What happened, and what you would do differently.
Attribute areas
Which of the seven this speaks to. Good evidence usually covers three or four.
Evidence held
Where the supporting material is, and whether you can still access it.
Strength
No evidence, early, developing, strong, mentor review requested, needs experience.
Next action
What would make this stronger, or what is missing.
Use it well
- Write entries within weeks of the work. A year later you will remember the outcome and not the reasoning.
- Grade the strength honestly. A matrix where everything is strong tells you nothing.
- Keep confidential detail out of it. Use a code name and keep the specifics elsewhere.
